PACS Services

PACS is dedicate to providing students with support and education for their personal and academic success through a variety of services. Find out more about services at PACS.

Although electronic mail (email) is not a confidential means of communication, with your permission, PACS staff may communicate with you via email to provide resources, referrals, psychoeducational materials, or discuss scheduling concerns. Responses to emails may take up to two business days. Email will not be used for therapeutic interventions or urgent matters. Email communications will be documented in PACS electronic medical records.

For students under 18 years old, we require parents or legal guardians to provide written consent by completing the Informed Consent Agreement before they are able to meet with a provider at PACS. To authorize a release of information or to request confidential information for students under the age of 18, we require parents or legal guardians to complete the Authorization for Release of Information/Request of Confidential Information form. These forms can be downloaded from the links below and returned to PACS via fax at (603) 862-0650, dropped off in person, or mailed to PACS at 3 Garrison Avenue, Room 306, Durham, NH 03824. If you prefer to receive the form to complete and sign electronically, please contact our receptionist at (603) 862-2090 to make this request.

If you have been asked by a student working with PACS?to provide supporting information for the purpose of a psychological assessment,?please click on the two links below to download and complete both questionnaires. We ask that you do your best to provide accurate and complete answers, and that you attempt to answer every item on the questionnaires. The questionnaires are designed so that you can click or type your answers directly onto the document. When both questionnaires have been completed, we ask that you return the forms to PACS via fax (603) 862-0650, mail to 3 Garrison Avenue Smith Hall, 3rd Floor, Durham NH 03824), or have your student return the completed forms to PACS. Finding a Provider Off Campus?

Many students find that frequent, long-term, or specialized services in the community are a more appropriate fit for their presenting problems and/or treatment goals. PACS staff are happy to assist students in finding community referrals, so please encourage your student to schedule an appointment at PACS.?
